Baptismal Permission Form

Use this form to obtain permission for any person who is under the age of 18 years old from their legal parent / guardian.

Dear parent or guardian, Your child has expressed an interest in being baptized at The Peninsula Pentecostals of Newport news, Virginia. It is our church policy not to baptize minors without their parents being present or without written consent from the parent/guardian and without the child's understanding of the significance of water baptism. We hope that you are in support of your child's decision and we would love for you to join us for this special occasion, along with any other family members who could attend. However, in the event that you were unable to be present for the baptism but give your consent for your child to be baptized, we ask that you please complete, digitally sign, and submit this permission form. I give my child permission to be baptized at The Peninsula Pentecostals.
